P0191 is triggered when the ECM detects that the fuel rail pressure sensor is reading a value outside its normal operating range. On Mahindra diesel engines, the high-pressure common rail system runs at 1,400–1,800 bar. If the sensor reports incorrect pressure, the ECM cannot control fuel injection accurately, causing misfires, poor power, and potential injector damage.
